NATIONAL DISABILITY INSURANCE SCHEME (NDIS)

GUIDE

Your Application Guide to Australia's Disability

Support System

Welcome to our easy and comprehensive guide on starting your NDIS journey.

As the leading NDIS provider on the Sunshine Coast, we’ve got you covered from starting to navigate the National Disability Insurance Scheme or seeking clarification on recent changes.

What is NDIS?

The National Disability Insurance Scheme

(NDIS) is a program initiated by the Australian Government, designed to provide financial assistance to individuals under the age of 65 with disabilities.

This support aims to cover the expenses

associated with necessary care and support services.

How does the NDIS work?

The NDIS offers funding and services to

eligible individuals with disabilities.

Participants create personalised plans,

implemented with provider assistance.

Funding covers various supports,

emphasizing choice and control for

participants in managing their funds and selecting providers.

How do I know if I'm eligible?

You must be under 65 years of age, be an

Australian citizen or hold a permanent visa, and have a significant and permanent disability.

NDIS assesses factors: functional impairment, support needs, disability impact. You’ll provide medical

evidence and support details.

How do I apply to the NDIS?

To begin, either fill out an access request

form on their website or reach out to them directly. Share details about your disability, any limitations you face, and the support you currently receive.

Be sure to include any relevant medical

evidence. Once approved, you’ll collaborate with the NDIS to craft a tailored plan suited to your needs.

How to create my NDIS plan?

Creating your NDIS plan involves discussions and assessments with an NDIS planner or Local Area Coordinator

(LAC).

Together, you’ll identify your goals and needs to decide on supports and services, tailored to you and your funding. The plan is reviewed yearly for any adjustments needed.

What's next after I have my plan?

After creating your NDIS plan, the next

step is to put it into action. This involves connecting with NDIS providers to

arrange the supports and services

outlined in your plan.

Seek help from your NDIS planner or LAC for guidance and adjustments during

implementation.

THE QUEST COLLECTIVE METHOD

Six Pillars. One Transformation.

Real change isn't random. It's built - pillar by pillar

At Quest Collective, we don't believe in band-aids or one-size-fits-all support.

We believe people are wired to grow when the right foundations are in place.

That's why everything we do is built on six simple, science-backed pillars - the same six things

that settle the nervous system, rewire the brain for hope, and turn "just getting by" into genuinely thriving.

Here's how we help our people transform (based on the best of neuroscience).

Mateship

Belonging is the foundation everything else is built on.

We're wired for connection - the brain literally settles and grows when we feel safe with the people around us.

Real mateship brings trust, belonging and a hit of the good stuff (dopamine, serotonin, oxytocin). No one transforms alone, and with us, no one has to.

Movement

Move the body, change the brain.

Regular movement lifts mood, sharpens focus and builds resilience that spills into every part of life.

A walk in the sun, a bushwalk, the gym, yoga, climbing - we help you find movement you actually enjoy, so it becomes energy and confidence, not a chore.

Mindset

The way you think shapes the way you live.

The brain can be trained to think in healthier, more hopeful ways.

Using practical, science-backed tools, we help shift unhelpful patterns, build self-belief and practise real gratitude and positivity. Small shifts, big changes over time.

Mastery

Confidence isn't given, it's earned through doing.

As you build real skills and tackle what once felt hard, the brain rewards the progress and motivation grows.

We focus on steady, achievable wins that upregulate dopamine and serotonin - so you feel capable, proud and ready for the next step.

Meaning

A life that feels worthwhile is one of the strongest drivers of wellbeing there is.

We help you reconnect with what matters - relationships, passions, values, the moments that bring joy.

It's the story you tell yourself that pulls you out of high-cortisol survival mode and into safety, motivation and resilience.

Mission

Purpose pulls you forward.

Being of service moves us beyond our own struggles and self-consciousness. A goal, a contribution, a reason to get up in the morning - it gives the brain direction and momentum.

We help you discover your mission and take real, practical steps toward it.

On a free, no-obligation call we'll get to know you and your support

needs, then build a custom plan to help your loved one build a life

they love. It's practical, straightforward, and we get results.
